Go to your iPhone or iPad's Settings > scroll down and tap Accounts & Passwords

Note: If you're on iOS 10, go to Mail > Accounts  

Select Exchange. (Could be called Work, Blue sky, Caregivers, etc)

Choose Delete Account.

Select Delete from my phone to confirm the removal of your old account, and Reboot your Phone.
